THE ROLE OF STAPHYLOCOCCUS AUREUS IN NASAL POLYPOSIS

Abstract

Nasal polyposis is associated with colonization of S. aureus, which secretes enterotoxine that acts as superantigen and induces formation of monoclonal IgE by analogy with severe eosinophilic inflammation. This article describes the current insights of features of inflammation caused by S. aureus. Further research aimed at a deeper study of the role of S. aureus in pathogenesis of nasal polyposis, as well as new methods of treatment that would prevent the recurrence of the disease and improve quality of life of the patients are needed.

CYTOKINES PROFILE IN BLOOD IN SEVERE ASTHMA PATIENTS WITH OBESITY

Abstract

Background. To study the cytokines profile in the peripheral blood of patients with severe asthma and obesity. Methods.139 patients with severe bronchial asthma have been examined, including 66 patients with severe bronchial asthma and body mass index (BMI) <30 (1st group), 73 persons with severe bronchial asthma and BMI >30 (2nd group). The control group was formed from 40 healthy volunteers. The respiratory function, the cytokine integral index and the levels of TNFα, IL4, IL6, IL2, IL10, IFN-γ, IL8, IL17 in the peripheral blood of patients in and out of an exacerbation were studied. Results. More significant systemic inflammation signs expressed in increased levels of TNFα, IL6, IL4, IL2 and IFN-γ were found in exacerbation period of the disease. In obese patients the increased level of proinflammatory cytokine IL17 in exacerbation period was estimated. Conclusion. We have demonstrated the presence of systemic inflammation in patients with severe BA, regardless of the presence of obesity, increased level of proinflammatory cytokine IL17 in patients with severe bronchial asthma and obesity in the period of exacerbation. It can be a reason of the low effect of steroid therapy in patients with BMI >30.

EPITHELIAL CELLS EXPRESSING TLR-2 LEVEL AND NOSE MUCOSA BIOCENOSIS IN ALLERGIC RHINITIS PATIENTS

Abstract

Background. Determination of local epithelial cells expressing TLR2 as an indicator of local immunity in allergic rhinitis (AR) patients with opportunistic species of staphylococci nasal mucosa colonization. Methods. Washed epithelium samples obtained from patients with seasonal AR (n=8) aged 19—42 years, and perennial AR (n=15) aged 19 —45 years, as well as a control group (20 patients) aged 19—45 years without allergic diseases were investigated. Epithelial cells expressing TLR2 receptors were determined by flow cytometry. Results. The level of epithelial cells expressing TLR2 receptor in patients with seasonal AR was almost in 1,9 times, in perennial AR group — in 1,7 times lower then in healthy individuals. In patients with perennial AR S. aureus was obtained in 96,0% (CI: 79,5—99,2), in association with Str. pyogenes in 29% (CI: 14,9—49,2), Neisseria spp. — in 63,0% (CI: 42,7—78,8). Seasonal allergic rhinitis was characterized by association of S. aureus and S. hemolyticus (70,0%, 44,4—85,8). Conclusion. Ratio of epithelial cells positive for TLR2 in nasal lavage from patients with AR was lower than in healthy volunteers. Indicators proportion of epithelial cells expressing TLR2 in nasal lavage in patients with seasonal AR during an exacerbation period was significantly reduced (1,7—1,9 times), in comparison with healthy volunteers. In AR patients with increased density of staphylococci strains in nasal mucosa increased local epithelial cells expressing TLR2 in nasal lavage was established.

THE ROLE OF NEBULIZED BECLOMETHASONE DIPROPIONATE IN THE TREATMENT OF BRONCHIAL ASTHMA

Abstract

Beclomethasone dipropionate (BDP, Clenil UDV) is a well-studied inhaled corticosteroid for the treatment of patients with bronchial asthma (BA). Randomized controlled trials have shown that nebulized therapy with BDP was similar in its efficacy and safety to inhalation therapy with budesonide or fluticasone. Preferred patients for the treatment with nebulized BDP are elderly patients with asthma, unable to correctly perform inhalation with portable devices, patients with steroid-dependent asthma, and patients with a temporary loss of asthma control.

ASMANEX TWISTHALER - THE DRUG OF CHOICE AMONG INHALED CORTICOSTEROIDS

Abstract

Goals. To reveal the clinical efficacy and safety of Asmanex in patients with moderate and severe bronchial asthma (BA), treated with inhaled and/or systemic glucocorticosteroids (GCS), depending on the severity and level of control. Methods. 40 patients (age 18 to 65 years) were treated with Asmanex with equivalent to prior therapy with inhaled corticosteroids (ICS) doses or in combination with systemic corticosteroids prior therapy (without increasing the dose of systemic corticosteroids) for 3 months. Efficacy was assessed before and after the treatment on a background of Asmanex treatment by the need of additional using of agonists of β 2-adrenoreceptors, severity of daytime and nighttime asthma symptoms, dynamics of spirometry parameters. The efficacy of Asmanex was compared with the original baseline therapy of other inhaled corticosteroids at equivalent doses using the new delivery system.

Abstract

The article presents the results of a prospective noncomparative study of use of carbamylated monomeric allergoid Lais ® for sublingual immunotherapy (SLIT) in patients with allergic rhinoconjunctivitis caused by pollen grasses. We suggest that SLIT with Lais ® sublingual tablets is an effective and welltolerated form of treatment.

RATIONAL PHARMACOTHERAPY OF CHRONIC URTICARIA

Abstract

The aim of this study was to reveal most clinically effective and economically expedient anti-H 1-receptor antagonist cetirizine generation (original or generic), to carry out the analysis of pharmaceutical equivalence of cetirizine generation anti-H-receptor antagonists manufactured by different companies. Materials and methods. Study is executed in design of simple open comparative prospective randomized research in parallel groups. The preliminary analysis of volume and structure of H-antihistamine cetirizine generation products retails for the purpose of definition of the most widespread trademarks was carried out 117 patients with chronic idiopathic urticaria (CIU) treated with various trademarks cetirizine 10 mg/day within 4 weeks were included in the study. Clinical efficiency criteria: dynamics of urticaria symptoms, change of index severity, remission period, days free from CIU symptoms. The pharmaceutical analysis included comparative studying of rate of dissolution and quantitative identification of medicinal substance. The pharmacoeconomic analysis was done by an «expense-efficiency» method. Results. All investigated drugs of cetirizine in a dose of 10 mg dayly significantly (p<0,001) reduced CIU symptoms. In mild CIU clinical efficiency of original and generic drugs had no significant differences but in severe CIU showed obvious superiority of an original drug (p<0,01). The greatest quantity of the patients reached pharmacological remission, received Zirtec and Cetrine. Coincidence of dissolution profiles with criteria of similarity for Zirtec, Cetrine and Parlazine and rate of substance liberation for Zirtec and Cetrine explained rapidity of achievement of therapeutic effect of investigated preparations. The best results from the point of view of economic efficiency were shown with Cetrine therapy. Use of original Zirtec was more expensive, but also more effective. Discussion. Results of the study demonstrated clinical efficiency of cetirizine family products belonged to different manufacturers depending on their bioavailability which had the following rating of efficiency: Cetrine > Parlazine > Cetirizine-Hexal > Zodak.

EVALUATION OF HUMORAL IMMUNITY IN PREGNANT WOMEN WITH ALLERGIC HISTORY

Abstract

Background. The aim of the study was to evaluate humoral immunity and efficacy of diet therapy in pregnant women with allergic history. Methods. Monitoring group comprised 118 pregnant women. The study group included 83 pregnant women who received dietary correction of the diet with the inclusion of the New Zealand goat milk. The comparison group consisted of 35 pregnant women, dietary recommendations, with the inclusion in the diet of the cow’s milk (CM). The content of total serum IgE was detected using the immunoassay analyzer using monospecific reagent. Allergenspecific IgE antibodies to CM, βLH, casein and soy protein were determined by enzyme immunoassay analyzer using commercial kits «Allergopharma». Contents of immunoglobulins G, A, M in the serum were determined by nephelometry on immunochemical analyzer IMMAGE. Results. Unlike women from the comparison group half of pregnant women had a core group of allergic diseases, the majority of women in both groups were observed burdened obstetric history, indicating a decrease in their reproductive function. Dysfunctions of the gastrointestinal tract were observed in 34,7% of pregnant women in both groups, and the frequency in women with an allergy was 2,3 times higher than in women not suffering from allergic reactions. High levels of total IgE in serum were observed in the patients with allergy, diseases of the gastrointestinal tract, as well as their combination. The concentration of allergen specific IgE antibodies to proteins in cow’s milk was higher in women with IgEmediated allergies. In pregnant women with allergies and diseases of the gastrointestinal tract serum IgG and IgA levels were decreased. Conclusion. Application of hypoallergenic diet with cow’s milk replacement for New Zealand goat milk, as well as the exclusion of cow’s milk or a restriction in the diet of pregnant women caused a reduction of total IgE in the blood compared to previous treatment. Allergenspecific IgE antibodies to milk proteins decreased in the majority of women while dietary treatment.
