ALLERGY TODAY. ANALYSIS OF NEW EPIDEMIOLOGICAL DATA

Abstract

The fact that the prevalence of allergy in the world is growing steadily has been known for a long time. this explains the need for regular screening of epidemiological studies. Qualitative diagnostics allows to identify the disease in its early stage. the timely selection of pharmaco- and pathogenetic therapy significantly reduces the risk of exacerbations of the disease and allergy debut. education and availability of skilled medical care largely enhances the level of diagnostics and treatment of allergic diseases.

GASTROINTESTINAL MANIFESTATIONS OF FOOD ALLERGY: PATHOGENESIS, CLINICAL FORMS AND CLUES TO THERAPY

Abstract

Food allergies (fa), defined as an adverse immune response to food proteins, effect up to 3-5% of the popula- tion in westernized countries, and their prevalence appears to be rising. a variety of mechanisms underlie the allergic reactions, not all of them run through the IgE sensitisation. the absence of sIgE in peripheral circulation characterize a group of non-Ig-Emediated disorders, such as food protein-induced enterocolitis, enteropathy and prococolitis. In case of eosinophilic esophagitis, gastritis and enterocolitis IgE sensitization arises in nearly 50%, therefore these disorders are classified as mixed-type. In some cases of gastrointestinal fa local IgE production, when switching to IgE synthesis occurs only in one organ, have been proved. although systemic investigation of this phenomenon is still lacking. Immune inflammation coursed by food allergens can involve nearly all organs and systems. this review focuses on gastroenterological manifestations of food allergy, except of broad field of gluten intolerance that demands a separate thorough reviewing.

Abstract

Epidermal barrier insufficiency results in skin dryness and fissures in different chronic dermatoses. Barrier dysfunction is due either to genetic problems or lipid deficiency or damage of tight junction proteins. Here we discuss various abnormalities of epidermal barrier in eczema and psoriasis which necessitate to prescribe pro- tective and moisturizing agents to restore skin barrier. We give our own practice data of using Bariederm cream and balm in combined therapy of dyshidrotic eczema, plantar eczema and palmoplantar psoriasis.

ASSESSMENT OF THE METHODOLOGICAL QUALITY OF RANDOMIZED CONTROLLED TRIALS PUBLISHED IN «RUSSIAN ALLERGOLOGY JOURNAL» IN 2009-2013 (PART 1)

Abstract

Background. To assess the methodological quality of randomized controlled trials (RCTs) published in «Russian Allergology Journal» (RAJ) in 2009-2013. Materials and methods. Retrospective analysis of 96 original publications was carried out. For 8 RCT the risks of biases were assessed using the methodology of the Cochrane Collaboration. Accuracy of statistical analysis was assessed in accordance with established in 2009 journal’s requirements (will be presented in next publication). Results. 96 articles were analyzed, 8 (8%) of them were identified as RCTs. All the RCTs have a high risk of the biases and major mistakes in the statistical analysis. Conclusion. The methodological quality of RCTs is insufficient and needs to be improved. We consider that improvement of trials’ planning should play the most important role. Collaboration with the experts in clinical trials’ methodology is strongly recommended. The analysis empowers researchers to consider existing experience and to improve methodological quality of RCTs, their relevance to international standards.

HIGH-DOSE SUBLINGUAL ALLERGEN IMMUNOTHERAPY: ADVISABILITY AND SAFETY

Abstract

Efficacy of the allergen immunotherapy (ait) in the treatment of allergic rhinitis and asthma has been proved in many studies and systematic reviews. Higher safety profile of sublingual ait (slit) relative subcutaneous ait (sCit) defines preferences of slit. although the results of the first study of slit were published about 25 years ago and a lot of allergenic products for slit appeared during this period, poor evidence base and a lack of direct comparative controlled studies on the efficacy of slit vs sCit support debate about what way of allergen administration is more effective. it is possible, that lower efficacy of slit showed in earlier studies explains for the use of regimes with low daily and cours doses of allergens. in a recent review of the eaaCi it was shown the dosedependent therapeutic effect of ait, and was noted that an increase of the maintenance allergen dose while sCit increases the risk of adverse events, vs high doses slit demonstrates good safety profile with a significant increase of efficacy. it is possible that future studies using high doses of standardized allergens for slit will show not only more high profile of slit safety, but also its higher efficacy vs sCit.

APPLICATION OF SERTACONAZOLE FOR SUPERFICIAL MYCOSES COMPLICATED BY BACTERIAL INFECTION

Abstract

Sertaconazole properties are discussed in the article. According to literature data application of the derivation of benzotiafene and imidazole solves a problem of resistance towards antimycotics. Sertaconazole has a wide spectrum of activity towards fungi & bacteria and also has antiinflammatory effect. The present examination of therapeutic efficacy included 33 patients who suffered from superficial mycoses complicated by bacterial infection. Levocetirizine was prescribed in case of dermatomikides. Both monotherapy by sertaconazole and combination with levocetirizine were high effective and well tolerable.

THE PRACTICE OF CLINICAL USE OF AZOXIMER BROMIDE FOR THERAPY AND PREVENTION OF RESPIRATORY INFECTIONS

Abstract

The review presents the results of various studying of basic properties of azoximeri bromidum (polyoxidonium). The experience of application of immunomodulator in patients with respiratory diseases was analyzed. polyoxidonium has expressed immunomodulatory activity (including interferonproducing), has nonspecific protective effect against a broad range of pathogens that are not based on direct inhibition of microorganisms, and on the regulation of immune function. in addition, the drug has antitoxic and antioxidant properties. in the acute period of the disease polyoxidonium is recommended to prescribe in combination with etiotropic therapy and during rehabilitation or prevention of infectious diseases as monotherapy.
