Treatment of urticaria: today and tomorrow
Abstract
Urticaria is a common disorder with diverse clinical presentations. Correct recognition of the different clinical patterns helps clinical assessment and treatment. The management of urticaria involves recognition of relevant disease associations, treatment of any identifiable external causes including infection, avoidance of drug, food and physical triggers and the appropriate use of pharmacological therapies. These can be divided into first, second and third-line approaches. The choice of treatment will be influenced by many factors including drug-licensing, safety, pattern of disease, its severity, pharmaco-economic considerations and patient preference. Possible new therapeutic approaches on the horizon include new H1 antihistamines, H4 receptor antagonists, a histidine decarboxylase inhibitor under development and biological agents that target histamine-releasing autoantibody production and function.

Historical prerequisites of development of the allergenspecificimmunotherapy method (to centenary of discovery)
Abstract
In connection with the approaching date of the centenary of the discovery by British researchers L. Noon and J. Freeman method of allergen-specific immunotherapy (ASIT) is a survey, which conducted a historical analysis of the significance of this event for the development of allergology. Studied a large number of literary sources, on the basis of which presents data on the background of the method ASIT, its essence, the authors. Compiled by a table which illustrates the basic milestones ASIT from pre-scientific stage before the modern state.

Oral allergenspecific immunotherapy and the way of its improvement
Abstract
Oral method of allergen-specific immunotherapy (ASIT) is found to be the safe and available for the patients with allergic diseases. In this review the mechanisms of oral tolerance formation and the results of the efficacy and safety of oral ASIT are presented. The modern dosage forms of oral allergovaccines are analyzed.

The peculiarities of cytokine status in children suffering from persistent allergic rhinitis during early stages of allergenspecific immunotherapy
Abstract
Background. To study dynamics of cytokine status during early stages of allergen-specific immunotherapy (ASIT) in children suffering from persistent allergic rhinitis (PAR).
Methods. 30 children suffering from PAR have received complete course of ASIT. Serum levels ofILL-4, -5, -6, -8, -12, -13, sIL-2R were studied in 0, 7, 30 and 90 days and have been analyzed differentially in groups with good/excellent and incomplete effectiveness of treatment.
Results. Good/excellent effectiveness of completed course of ASIT has been demonstrated in 17/30 (56,7%) children, partial effectiveness has been demonstrated for 13/30 (43,3%) ones.The decreased IL-12 and IL-6 levels and the increase of sIL-2R production were described for good/excellent results of treatment, but insufficient effectiveness was characterized by delayed dynamics of these cytokines. IL-8 level was increased in both groups but decreased effectiveness was associated with early start and long duration of it. IL-4 production was decreased in the good/ excellent group while on the second one changes of cytokine level were absent. IL-13 and IL-5 synthesis were increased in both groups.
Conclusion. The effectiveness of ASIT in PAR patients depends on synthesis dynamics of ILL-4, -6, -8, -12, sIL-2R at the early treatment stages.

Assessment of antimicrobial peptides(alpha-defensins) in atopic dermatitis and pyodermia patients blood
Abstract
The purpose of research. The purpose was to investigate alpha-defensin levels in the circularly neutrophiles of atopic dermatitis and pyodermia patients in comparison with healthy donors.
Materials and methods. 27 patients with atopic dermatitis, 31 patients with pyodermia in comparison with 17 healthy donors were investigated. Intracellular alpha-defensin levels in neutrophils in the peripheral blood were estimated by flow cytometry with mouse anti-HNP-antibodies (Hy cult biotechnology).
Results. The alpha-defensins level in patients with atopic dermatitis and pyodermia was reduced in comparison with healthy donors. More expressed decrease of alpha-defensins level was obtained in patients with pyodermia and sever atopic dermatitis with skin infection.
Conclusion. Thus, a decrease of alpha-defensins levels in atopic dermatitis and pyodermia patients, possibly, is a marker of the chronic bacterial inflammation and may cause chronic bacterial skin disease, St. aureus colonizations and tolerance to the therapy.

The modern aspects of atopic dermatitis pathogenesis in children
Abstract
The article analyzes the results obtained by modern Russian and foreign researchers who focused on the pathogenic factors responsible for the appearance and the development of atopic dermatitis. The reasons for skin dryness appearance, the mechanisms of itch development, the immune system role in allergic skin inflammation: the involvement of lymphocytes, antigen presenting sells, mast cells, basophiles and eosinophiles into the pathological process have been thoroughly observed. The causes contributing to atopic dermatitis development and triggering disease aggravation have been described.

Recurrent upper respiratory tract infections and immune defi-ciency in children
Abstract
The lecture is devoted to results of clinical and immunological investigation of 614 children with 20 forms of primary immunodeficiency states and more than 8000 children with «frequent infections». The observations were performed in children unit of the Institute of Immunology in the years 1981-2010. The main features of primary immunodeficiency and secondary immunodeficiency and the term «immunocompromised child» are analyzed in details.

Relationship between allergic rhinitis and bronchialasthma

The aim of the review is to discuss different aspects and mechanisms of the relationship between allergic rhinitis and asthma. Therapeutic strategy has to include the treatment of both diseases. Modern treatment of allergic rhinitis may improve control of concomitant asthma.

THE EFFICACY AND SAFETY OF FOSTER IN PATIENTS WITH BRONCHIAL ASTHMA IN ROUTINE CLINICAL PRACTICEon behalf of the Research Group «ESTAFETA» to assess the efficacy and safety of the drug «Foster» in patients with asthma in routine clinical practice
Abstract
Background. Evaluation of the efficacy and safety of the drug Foster (fixed combination of beclomethasone di-propionate 100 mcg and formoterol 6 micrograms per single dose) in metered-dose aerosol inhaler technology Modulite® in patients with bronchial asthma (BA).
Methods. Prospective non comparative multicenter open observational study. Study group consisted of 120 practicing physicians from the 92 hospitals in 21 cities of Russia. Analyzed 557 case report forms of patients with moderate persistent asthma treated with Foster during 12 weeks who needed to correct the basic therapy (according to asthma control criteria GINA 2006).
Results. The effectiveness of treatment with Foster was observed in 555 (99,6%) of 557 patients, who showed improvement in lung function, decreased severity of symptoms, reducing the need for symptomatic therapy compared with baseline and improved asthma control in general.
Conclusions. In most cases, control over moderate and severe persistent asthma can be improved by the combined therapy of inhaled steroids (ICS) and в2-agonist long-acting (LABA). In this case, Foster is a drug of choice with a fixed combination of ICS and LABA. Extra-fine form of the drug (thanks to innovative technology Modulite®) is evenly distributed in central and peripheral airways, providing good therapeutic effect of using of low doses of ICS and reducing the likelihood of side effects.

Treatment of respiratory virus infections in patients with perennial allergic rhinitis
Abstract
Цель работы. Оценка эффективности и переносимости цетиризина у больных КАР легкой степени тяжести интермиттирующего течения, с сенсибилизацией к бытовым аллергенам, страдающих ОРВИ. Материалы и методы. В исследовании приняли участие 41 пациент с КАР, болеющие ОРВИ (23 женщины и 18 мужчин), в возрасте от 18 до 56 лет (средний возраст 32,5±1,2), 1-ю группу составили 22 (53,6%), в схему лечения ОРВИ которых был включен цетиризин в дозе 10 мг один раз в сутки в течение 20 дней. 2-ю группу составили 19 (46,3%) пациентов, в схему лечения которых не были включены АГП. Оценку результатов проводили на 5-, 12-, 20-й день терапии. При каждом визите оценивалась в баллах степень выраженности и тяжести клинических признаков риноконъюнктивального синдрома и выраженность симптомов общей интоксикации.
Результаты. Прием цетиризина способствовал уменьшению не только выраженности симптомов КАР, но и симптомов ОРВИ. У пациентов, принимавших цетиризин в дополнение к базисной терапии, быстрее купировались симптомы ОРВИ, не требовалось усиление базисной терапии, не отмечено осложненного течения заболевания.
Заключение. Показана высокая терапевтическая эффективность и хорошая переносимость цетиризина, что позволяет рекомендовать его использование в комплексной терапии ОРВИ у больных КАР.

Prenatal risk factors of atopic dermatitis inchildren and possibilities of their correction in antepartum period
Abstract
У 118 беременных женщин оценивалось состояние здоровья и течение беременности. основную группу составили 83 беременные женщины, находившиеся под нашим наблюдением, получавшие диетическую коррекцию рациона питания, посещавшие занятия по подготовке к родам и к грудному вскармливанию будущих младенцев. в группу сравнения вошли 35 беременных женщин, соблюдавшие обычные диетические рекомендации, предлагаемые в женских консультациях, с включением в рацион коровьего молока и не посещавшие подготовительные занятия. проведенный анализ состояния здоровья наблюдаемых женщин показал, что половина беременных женщин основной группы имела аллергические заболевания. у большинства женщин в возрасте до 35 лет отмечался отягощенный акушерский анамнез. применение гипоаллергенной диеты с заменой коровьего молока на козье, так же как исключение коровьего молока или его ограничение в рационе беременных женщин, вызывало снижение содержания общего IgE в крови женщин по сравнению с показателями до лечения. наиболее выраженное снижение содержания общего IgE отмечалось в подгруппах, получавших гипоаллергенный рацион с козьим молоком или находившихся на безмолочной диете. у большинства наблюдаемых женщин отмечалась положительная динамика в сторону снижения показателей аллергенспецифических IgE антител к молочным белкам на фоне проводимой диетотерапии. у женщин, употреблявших козье молоко, длительность грудного вскармливания более 6 мес была в два раза выше. Развитие атопического дерматита и гастроинтестинального синдрома, как на естественном, так и на искусственном вскармливании, чаще наблюдалось у тех детей, чьи матери страдали аллергией и употребляли коровье молоко в течение беременности и во время кормления грудью.
