FUNGAL ALLERGY

Abstract
Fungal allergy is discussed in this review. Aerobiological, epidemiological data are presented. Immunobiology of fungal (mould and yeast) allergens and mechanisms of fungusinduced allergic diseases also summarized in the review. Fungi are ubiquitous in environments. Mold spores are found both indoors and outdoors. Most fungi are not pathogenic to healthy humans while some cause diseases. Molds and other fungi produce allergens, irritants, and toxic substances (mycotoxins)

POLLEN ALLERGY IN SARATOV REGION

Abstract
Background. To assess time trends in prevalence of pollen-related allergy in Saratov region during the last 3 decades . Materials and Methods. Agro-climatic, aerobiological monitoring (pollens of trees, grass, weeds were collected using gravimetric method and counts were done using a standardized protocol). Concentration of salts of heavy metals determined by means of mass - spectrometry in the pollen grains from plants, growing in city and village («dirty» and «clean» areas). Epidemiological (screening with questionnaire), clinical examination, skin tests were used to define the clinical diagnosis. Results. The pollen map of Saratov region has been changed because of climate, melioration and expansion of the Cyclachena xanthyfolia and other weeds. Detection of Pb, Zn,Cu,Ni in pollen grains collected from these plants in «dirty» areas showed that they accumulate heavy metals. The prevalence of pollinosis in 1977 in Saratov region was 1,4% , in 2002 -12,1%, in 2007-2,3%. Among all patients with pollen allergy 4,51% (1977), 5,2% (2002) and 16,0 % (2007) had positive skin tests with trees pollen allergens, 16,0, 36,7 and 29% -with grass pollen allergens and 78,5, 88,9 and 86,4 % -with weeds pollen allergens. Conclusion. Our results established increasing of pollinosis in Saratov region between 1977 and 2002, and there was a tendency towards stagnation in current symptoms of pollen-related allergy. High frequency of weeds allergens sensitization could be explained by allergenicity and prolonged blossoming period of new plants in this region.

In this review the main physiological effects of nitric oxide in the human body have been considered, as well as its pathophysiologic role in bronchial asthma. Methods for determination of nitric oxide in exhaled air have been described. The importance of determining the level of nitric oxide in patients with bronchial asthma has been analyzed.

Abstract
В статье рассмотрены вопросы перекрестной реактивности между групповыми пыльцевыми аллергенами, принципы выбора оптимальной аллерговакцины для проведения аллерген-специфической иммунотерапии (АСИТ ), обсуждается феномен перекрестной пищевой непереносимости у больных поллинозом. Феномен перекрестной реактивности между родственными аллергенами обусловливает не только эффективность АСИТ при использовании лишь основных (мажорных) аллергенов, но и снижение перекрестной пищевой непереносимости на фоне АСИТ пыльцевыми аллергенами. Т ем не менее,при наличии сенсибилизации к нескольким родственным аллергенам оптимальным является применение микст-аллергена с наиболее полным антигенным представительством, приближенным к естественным условиям экспозиции.

Multiple faces of bronchial asthma

Abstract
The analysis of publications and results of original investigations concerning the pathogenesis and clinical course of bronchial asthma is performed. B ronchial asthma is a markedly heterogeneous disease, the heterogeneity is due to the differences in severity cause, phases, clinical pathogenetic variants, associations with the other allergic diseases, COP D, diseases of other organs, influence of different allergens and infectious agents. T hus, the effective treatment plan should be based also on attention to individual features of the patient and include both drugs and methods of non-drug treatment.

THE CLINICAL DISPLAYS AND THE FUNCTIONAL ACTIVITYOF RIBOSOMAL GENES IN ASTHMATIC CHILDREN

Abstract
Background. To analyze the interrelations between the parameters of the functional activity of ribosomal genes and some clinical features in asthmatic children. Material and methods. 78 asthmatic patients were included in the study. Standard methods of production and colouring of cytogenetic preparations were used. The functional activity of ribosomal genes was defined on five-point system in arbitrary units. Statistical data processing was spent with the help of applied programs Statistica 6.0. Results. The data about the level of the functional activity of ribosomal genes of asthmatic children (18,57 arbitrary units) were obtained. In all patients the general functional activity of ribosomal genes parameters were correlated with the number of leukocytes and the functional activity of ribosomal genes localized in GHchromosomes parameter -with the level of immunoglobulin E. We constructed the discrimination model according to the disease severity index which included 4 significant parameters: bands number, disease manifestation age, blood sedimentation rate and active ribosomal genes copies number. Conclusion. Our data give evidence about the interrelations between the functional activity of ribosomal genes parameters and some clinical features in asthmatic children. The number of active ribosomal genes copies is considered to be one of the significant features of disease activity.

THE EXPERIENCE OF ATOPIC DERMATITIS TREATMENT WIT«LAENNEK» («JAPAN BIOPRODUCTS INDUSTRY CO., LTD», JAPAN) IN RUSSIA

Abstract
Цель исследования. Оценить клиническую эффективность и безопасность препарата Лаеннек в комплексной терапии больных АтД. Материалы и методы. Для проведения исследования была отобрана группа из 60 пациентов с АтД среднетяжелого (n=42) и тяжелого течения (n=18), 39 женщин и 21 мужчина, в возрасте от 18 до 52 лет (средний возраст 28±4,2). Пациенты с подтвержденным диагнозом АтД в стадии обострения были рандомизированы на 2 группы методом случайной выборки. Пациенты группы 1 (опытная группа, n=30), 20 женщин и 10 мужчин, средний возраст 25±3,2 лет, получали стандартную терапию АтД за исключением системных глюкокортикостероидов (ГКС): топические ГКС, индифферентные смягчающие и увлажняющие средства, антигистаминные препараты и препарат Лаеннек. Пациенты группы 2 (контрольная группа n=30), 19 женщин и 11 мужчин, средний возраст 30±1,4, получали только стандартную терапию АтД за исключением системных ГКС. Результаты. При сравнении клинической эффективности лечения в обеих исследуемых группах индекса SCORAD и IGA (их средние значения на 0< и 28<й дни лечения) у больных АтД, получавших стан< дартную терапию и препарат Лаеннек (группа 1) и стандартную терапию (группа 2), выявлены достоверные различия (p<0,05). Разность по индексу SCORAD и IGA была больше в группе 1 по сравнению с группой 2. Выводы. Применение препарата Лаеннек в комплексной терапии АтД приводит к более выраженному уменьшению основных клинических симптомов АтД по сравнению с применением стандартной терапии. Препарат Лаеннек безопасен, хорошо переносится больными АтД и может быть рекомендован для лечения обострений хронического АтД в составе комплексной терапии.
