Abstract

UV exposure shows the immunosuppressive effect on T-cells in skin, causing various disturbances in DNA structure. The human epidermis basal layer is extremely susceptible to different types of UV. LI EMR EHF reveals a stimulating effect on TNFa production. LI EMR EHF RF can cause a brain tissue autoimmune antibodies production and in some cases leads to genotoxic effect.

ATOPIC DISEASES WITHOUT SYSTEMIC SENSIBILIZATION

Abstract

The diagnosis of allergic disease related to the group of immediate hypersensitivity involves confirmation of IgE sensitization: positive skin tests with the suspected allergen and/or detection of allergen-specific IgE. Research over the past decades showed that IgE synthesis may occur exclusively in the tissues of the affected organ and derived allergen-specific antibodies may not enter the systemic circulation. In contrast to «atopy», this concept of local sensitization was termed «entopy». local production of IgE was detected in more then 47% of adults previously diagnosed as having non-allergic rhinitis. besides the nasal mucosa, local sensitization was found in lymphoid tissues of nasopharyngeal ring, bronchial mucosa in some patients with non-atopic asthma, esophageal mucosa in patients with eosinophilic esophagitis. The processes of local immune response is currently studied and probably one day the accumulated data will swing our understanding of atopic sensitization, that will be reflected in classification and therapeutic approaches.

Abstract

Pollen, the male gametophyte of flowering plant species, is one of the most predominant sources of environmental allergens, and a significant cause of allergic disease. This review discusses the nature of pollen proteins as allergens, their effect on the human immune system. The influence of cross-reactive pollen allergens to the incidence of food sensitivities is also discussed. Flowering plant species that posses allergenic pollen, identified with allergens cloned from these species, are also discussed.

Abstract

In this article the problems of improving the control of bronchial asthma through optimization of bronchodilator therapy are discussed. Cholinergic mechanisms in the pathogenesis of asthma and the possibilities of use of long-acting m-anticholinergic - tiotropium (via liquid inhaler R espimat) - in the treatment of this disease are shown. Pharmacological properties of bronchodilators are discussed to identify the main groups of patients who need tiotropium therapy.

CLINICAL AND IMMUNOLOGICAL FEATURES OF ALLERGIC DISEASES AMONG PROFESSIONAL ATHLETES (CYCLING)

Abstract

Background. To study the epidemiological aspects, clinical and immunological features, diagnostic possibilities of allergic diseases among professional athletes in the Samara region for futher improvement of treatment and quality of life. Methods. Sociological and special questionnaires, history data, retrospective analysis of case records, clinical,, allergological and immunological methods were used in the study. the object of the study was a group of 35 athletes: 18 boys and 17 girls aged from 15 to 20 years old. Athletes included in the study were the members of the Russian youth national cycling teams titled as masters of sports. Results. Among sportsmen from the research group, professionally engaged in Cycling the prevalence of allergic diseases was 25,7% (p<0,05), in 7 people they were detected for the first time (20%, p<0,05). In the structure of allergic diseases prevailed respiratory symptoms - in 8 people (22,8%, p<0,05). In 9 people serum immunoglobulin Ig E level was 170 - 1500 g/l at (25,7%, p<0,05). Conclusion. the results of the study didn’t show any negative influence of the cycling sports to athlet’s health. the efficiency of medical care to athletes with allergic diseases depends on the coherence of the primary care physicians and allergists-immunologists.

Abstract

Bronchial asthma (BA) - a global public health problem worldwide. All treatments aimed at improving the control of symptoms of the disease, prevention of exacerbations and improving the quality of life for patients. The use of inhalation therapy is the basis for the treatment of all bronchoobstructive diseases. The effectiveness of treatment depends not only on the quality of the inhaled drug, but also by means of delivery of the drug into the bronchi. Easyhaler - a new dry powder inhaler, inhalation activated enables precise dosing of the drug. According to various studies, the method of delivery of drugs has proven high efficiency, ease to use and adherence of patients to the unit.

PREVALENCE OF ALLERGIC DISEASES SYMPTOMS AMONG CHILDREN OF THE GRODNO REGION (REPUBLIC OF BELARUS)

Abstract

Background. To study the prevalence of symptoms of allergic diseases among school students of the Grodno region (Republic of Belarus). Methods. Researches conducted a questioning method by means of the Russian version of the standardized «International Study of Asthma and Allergy in Childhood» program (ISAAC). Results. Results of questioning of 1787 schoolchildren showed that the wheezing, within the last 12 months was registered in 9,3% of children of the first classes (DI: 7,5-11,5%) and in 9,1% of children of the eighth classes (DI: 7,5-11%), rhinitis symptoms without cold or flu - in 29,7% of children of the first classes (DI: 26,7-32,85%) and in 47,2% of children of the eighth classes (DI: 44-50%), symptoms of atopic dermatitis - in 8,2% of children of the first classes (DI: 6,6-10%) and in 1,9% of children of the eighth classes (DI: 1-2,97%). Frequency of clinically verified diagnosis of bronchial asthma at schoolchildren at the age of 6-7 y.o. and at the age of 13-14 y.o. was 3,9% (DI: 3-4,9%), allergic rhinitis - 5,6% (DI: 4,6-6,8%), atopic dermatitis - 6,2% (DI: 5,2-7%). Conclusion. Results of the study confirm the need of carrying out further epidemiological researches for the purpose of earlier identification of patients with allergic diseases.

NEW OPPORTUNITIES FOR THE TREATMENT OF UNCONTROLLED ASTHMA IN RUSSIA

Abstract

Bronchial asthma (BA) is one of the most socially significant diseases of the respiratory system. The control over the symptoms of BA is achieved less than in 50% of cases even in highly developed countries. The reasons for the lack of control are heterogeneous. One of the most important reasons for the lack of control over symptoms BA in Russia is the unavailability of highly effective drugs for socially unprotected layers of the population. The solution of this problem could be the creation of Russian analogs for asthma treatment. A new Russian product - a combination of topical corticosteroids and DBA (fluticasone/salmeterol) - Suticase-native is going to appear on the Russian market. A study to assess the efficacy and safety of the new product was carried out on the base of three Russian scientific centers. This trail compared a new drug Suticase-nativ (salmeterol 50 mcg plus fluticasone propionate 250 mcg/dose), powder for inhalation (produced by «Nativ» - company, Russia), with its famous counterpart - a Seretide accuhaler diskus (salmeterol 50 mcg plus fluticasone propionate 250 mcg/ dose), powder for inhalation dosed, production Glaxo Operates UK Ltd, in patients with bronchial asthma. The results of the study showed high efficiency and safety of Suticase-Nativ, comparable to Seretide.

ANTIHISTAMINES IN ACUTE CASES OF ALLERGIC DISEASES

Abstract

Histamine is the main mediator of both true allergic and pseudoallergic reactions. Acting with histamine receptors it causes the appearance of the main symptoms of allergic diseases in various organs and tissues, so the search for effective and safe antihistamines continues for a long period of time. currently, there are antihistamines of 1 and 2 generation, which differs primarily by the ability of passing through the hemato-encephalitic barrier. despite the fact that for the majority of allergic diseases, the drugs of choice are nonsedating antihistamines 2 nd generation, which have no sedative effect, in some cases, antihistamines of the 1 st generation are not only preferable, but also indispensable. the article provides the analysis of the clinical situations in which antihistamines of the 1st generation are the tools of choice on the example of clemastine (tavegil).

Abstract

The article presents the current understanding of the genomic and nongenomic mechanisms of action of glucocorticoids, outlined data which cast doubt on the assertion of the positive role of nongenomic effects of this group of drugs for the treatment of patients suffering from atopic dermatitis, and the necessity of further experimental and clinical studies of corticosteroids with regard to the mechanisms of their action. Reasoned application of topical corticosteroids that have proven a high index of efficacy/safety with a wide range of dosage forms, which allows to optimize therapy for a particular patient.

LOW-FREQUENCY ULTRASOUND THERAPY WITH LEVOCETIRIZINE IN PATIENTS WITH ACUTE INFECTIOUS RHINITIS

Abstract

This article represents the results of research of low-frequency ultrasound with levocetirizine in patients with acute infectious rhinitis (AIR). Low-frequency ultrasound therapy with levocetirizin effectively reduces rhinitis symptoms (nasal obstruction, sneezing, rhinorrhea) duration, demonstrates anti-inflammatory activity and reduces of topical decongestants requirement in AIR patients. Effectiveness of this therapy was estimated by objective and subjective methods.
