Allergy to egg whites: diagnosis, symptoms (clinical features), prognosis
Abstract
Allergic reactions to hen’s egg are rather a common problem, especially in infants. Hen’s eggs contain a few protein components which may serve as food allergens. They are 4 egg-white proteins: ovomucoid (Gal d 1, 11%), ovalbumin (Gal d 2, 54%), ovotransferrin (Gal d 3, 12%), lysozyme (Gal d 4, 3,4%) and one egg-yolk protein - alpha-livetin (Gal d 5). Ovomucoid (OVM) is the dominant allergen in hen’s egg. The allergenicity of egg whites depends mostly on their resistance to heat and digestive enzymes. A skin prick test size of 5 mm in children younger than 2 years old and 7 mm in children older than 2 years had a positive predicative value (PPV) of 100%. However, patient questioning and skin testing cannot be the only proof of food allergy. There exists a positive correlation between the growing levels of eggs-pecific IgE and the probability of allergic symptoms occurrence. Testing for sensitization to different hen’s egg proteins (molecular diagnostic) may assist in marking off allergy reaction to raw eggs from that one to heat-treated eggs. Questionable cases call for carrying out challenge tests which are not certified in Russia. The recent studies have demonstrated the safety of measles vaccination in children with proven eggwhite allergy. Currently, oral, sublingual and epicutaneous immunotherapy for food allergy to egg-whites has been extensively investigated in clinical trials.

Features of fungal sensitization in patients with asthma and cystic fibrosis: the results of a prospective study
Abstract
Background. To estimate the frequency of fungal sensitization and allergic bronchopulmonary aspergillosis in patients with asthma or cystic fibrosis. Methods. The study included 205 patients with asthma and 190 patients with cystic fibrosis. For detection of fungal sensitization specific IgE antibodies in the blood serum with «AllergoELISA-specific IgE» test system and biotinylated fungal allergens were determined. Mycological examination included microscopy and cultural study of respiratory biomaterials. According to the indications, a CAT scan of the chest was performed. Results. The incidence of fungal sensitization in patients with asthma was 35,6%, CI [29,1%; 42,6%], in patients with cystic fibrosis - 56,8% [49,5%; 64%]. In patients with severe asthma the main fungal allergens were Alternaria spp. (70%) and Aspergillus spp. (60%); in patients with cystic fibrosis: Candida spp. - 73%, Alternaria spp. - 34%, Aspergillus spp. - 27%. The incidence of allergic bronchopulmonary aspergillosis (ABPA) in patients with asthma was 5,3%, in patients with cystic fibrosis - 5,7%. Conclusion Early detection of ABPA in patients with asthma and cystic fibrosis is very important as it changes the therapeutic measures of patients’ management and prevents progression of bronchiectasis and respiratory failure. Determination of specific Aspergillus spp. IgE antibodies in vitro is a necessary stage of diagnosis of ABPA in patients at risk.

What is аsthma-COPD Overlap syndrome (ACOS), diagnostics and treatment
Abstract
175 patients were examined: 78 patients with bronchial asthma (BA), 39 with Asthma-COPD Overlap Syndrome (ACOS), 38 with chronic obstructive pulmonary disease (COPD) and 20 healthy individuals. A general clinical and laboratory examination was conducted, α1-antitrypsin (AAT), IgG and IgA to four bacterial antigens were determined, and pulmonary function test was studied. The clinical and spirometry data (characterizing the ACOS) were received. Negative association of AAT with respiratory function parameters in patients with ACOS, high levels of IgG antibodies to bacterial antigens and the lack of these connections in patients with BA and COPD allows us to consider ACOS to be independent nosological form related with microbial inflammation and marked by the AAT.

Subcutaneous immunoglobulin for treatment of patients with primary immunodeficiencies: review of the literature and personal experience
Abstract
Substitution with normal human immunoglobulin is a golden standard of primary immunodeficiencies therapy. Subcutaneous immunoglobulins, especially delivered via «rpid push» method are gaining popularity among patients and treating physician since at home infusions allow to reduce treatment costs and increase patients’ quality of life. Here we report first Russian experience of subcutaneous infusion of Gamunex®-C immunoglobulin, in four patients with primary immunodeficiencies, performed by their parents via «rapid push» method at home. No systemic reactions have been reported, Gamunex®-C treatment prevented severe infections and allowed adequate average serum IgG levels (10,1 g/l while on subcutaneous infusions in comparison with 8,5 g/l while on intravenous immunoglobulin).

Place of levocetirizine in the treatment of allergic rhinitis
Abstract
Levocetirizine is an effective Hj-blocker in complex treatment of various allergic patients. Allergic rhinitis is often part of a systemic allergic inflammation associated with other allergic diseases, therefore systemic antihistamines are the drugs of choice in it’s treatment.

Clinical experience of bilastine treatment in patients suffering from allergic diseases
Abstract
The results of clinical observation of 60 patients with urticaria (n=30) and perennial allergic rhinitis (n=30) aged from 12 to 61 years old are presented. All patients were treated with bilastine 20 mg once a day during 30 days. The assessment of urticaria activity score (UAS, UAS7), nasal and non nasal symptoms severity score of allergic rhinitis has been performed before, 2 weeks and 4 weeks of the treatment. The majority of patients demonstrated control of disease symptoms. The efficacy and safety of bilastine 20 mg in chronic spontaneous and acute urticaria as well as allergic rhinitis was shown.

Irrigational therapy in complex treatment of patients with allergic rhinitis
Abstract
Today allergic rhinitis is one of the most widespread diseases of a cavity of a nose. Irrigational therapy promotes considerable reduction of allergens at surfaces of a mucous membrane of upper airways and leads consumption of medicines at reduction in patients with allergic rhinitis.
