Intranasal corticosteroids and nonsedating antihistamines are the drug classes most often prescribed
to treat allergic rhinitis (AR). Treatment guidelines recommend a combination of these agents for moderate to severe AR. Objective: To compare the efficacy of mometasone furoate (MF) nasal spray plus desloratadine (DL)
with that of monotherapy with the individual agents in 6-16 years old patients with perennial AR.
Methods. A prospective, randomized, open label, parallel group study of the efficiency of MF nasal spray plus DL and of monotherapy with MF or DL in children with persistent allergic rhinitis aged 6-16 in a 12week period
was conducted. The efficacy of therapy was evaluated by level of markers of an allergic inflammation, indicators
of a bronchial hyperresponsiveness and by dynamics of clinical symptoms.
Results. Combined therapy (DL+MF) restores balance of cytokines: increase the Тh1 activity and decrease the Т2 activity. Combined therapy found to be effective in the majority of AR patients.

Combined therapy with MF and DL in the treatment of perennial AR in children has clinical advan60</p></abstract><trans-abstract xml:lang="ru"><p>Топические глюкокортикостероиды и неседативные антигистаминные препараты 2-го поколения наиболее часто назначаются для лечения персистирующего аллергического ринита (ПАР ). В рекомендациях
содержатся указания на возможность их совместного назначения при среднетяжелом и тяжелом ПАР .
Цель. Изучить клиническую эффективность курсовой терапии аллергического ринита у детей мометазоном фуроатом, дезлоратадином и их комбинацией, а также влияние этой терапии на уровень маркеров
аллергического воспаления и гиперреактивность бронхов.
Материалы и методы. Проведено проспективное, рандомизированное, открытое исследование в параллельных группах эффективности 12-недельной терапии дезлоратадином и мометазона фуроатом при совместном назначении и в монотерапии ПАР у детей в возрасте от 6 до 16 лет. Эффективность терапии
оценивалась по уровню маркеров аллергического воспаления, показателям бронхиальной гиперреактивности (БГР ) и динамике клинических симптомов.
Результаты. У пациентов с высокой БГР на фоне проводимой терапии наблюдалось менее выраженное
снижение общего IgE, интерлейкина-4 (ИЛ -4) и sICAM-1, чем в группе с нормальной и умеренной БГР .
Комбинированная терапия более эффективно восстанавливает цитокиновый баланс за счет повышения
активности Т h1- и снижения активности Т h2-лимфоцитов. Комбинированный вариант терапии позволил
быстрее, уже к концу первого месяца, купировать у большинства пациентов такие симтомы ПАР , как чихание, ринорея, заложенность носа, зуд в носу, а также симптомы, характеризующие качество жизни
пациентов (снижение дневной активности, нарушение способности к обучению).
Заключение. Комбинация мометазона фуроата и дезлоратадина при лечении ПАР у детей имеет клинические преимущества и способствует более выраженному снижению уровня маркеров аллергического
