The age of infants was 2, 5—9 months. The duration of observation was 3—12 months. Complex therapy has been conducted over the infants and it contained antiallergenic treatment and diet therapy with hypoallergenic baby formula. The majority of patients showed improvement due to applied treatment. The first group’s infants with positive clinical dynamic showed decrease of specific IgG and IgE to cow’s milk protein, casein, β-LH and soy’s protein. The infants of the second and third groups also showed decrease of specific IgG and IgE to cow’s milk protein, casein, β-LH, soy’s protein and goat’s milk at clinical remission. All the infants with positive clinical effect showed trend to decrease of IL12, IL13, sICAM1 and sEselectin’s concentration in the blood stream, that can indicate on leukocyte adherence inhibition and cell migration to the area of inflammation. Based on the results of research, we can conclude that replacement of one product with another one by taking into account their clinicalimmunological effectiveness as the method of treatment the of atopic dermatitis patients let us optimize infant diet therapy.</p></abstract><trans-abstract xml:lang="ru"><p>Целью работы была оценка клинико-иммунологической эффективности диетотерапии с использованием гипоаллергенных формул у детей с атопическим дерматитом (АтД) первого года жизни. Под наблюдением в амбулаторных условиях находились 167 детей с АтД, вызванным аллергией к белкам коровьего молока (БКМ). Б 1-ю группу вошли 52 ребенка, получавшие смесь на основе цельного козьего молока, во 2-ю — 56 детей, употреблявшие гидролизаты белка коровьего молока, в 3-ю — 59 пациентов, потреблявших смеси на основе изолятов сои. Возраст обследованных детей составил 2,5—9 мес. Длительность наблюдения за больными составила 3—12 мес. Всем детям проводилась комплексная терапия, включавшая противоаллергическое лечение и диетотерапию с использованием гипоаллергенных детских формул. У большинства больных отмечалась положительная динамика от проводимого лечения. У детей с позитивным клиническим эффектом, в 1-й группе больных, отмечалось снижение показателей аллерген-специфических IgG и IgE к БкМ, казеину, β-ЛГ и соевому белку. У детей второй и третьей группы при достижении клинической ремиссии также наблюдалось снижение специфических IgE и IgG антител к БкМ, β-лактоглобулину (β-ЛГ), казеину, белку сои и козьему молоку. У всех детей с положительным клиническим эффектом наблюдалась тенденция к снижению концентрации в крови IL-12, IL-13, sICAM-1 и sE-selectin, что может свидетельствовать о торможении стадии лейкоцитарной адгезии и миграции клеток в очаг воспаления.
