In recent years, not only an increase in the incidence, but also an increase in the prevalence of severe forms of asthma has been noted in all countries of the world. The effect of the sensitization spectrum on the severity and level of asthma control has not been studied enough.</p> <p><bold><italic>AIM</italic></bold><italic>:</italic> Evaluate the importance of sensitization spectrum to aeroallergens in severe asthma patients as indicators for disease exacerbation risk.</p> <p><bold><italic>MATERIALS AND METHODS</italic></bold><italic>:</italic> Single-center cross-sectional study at North-Western State Medical University n.a. I.I. Mechnikov. Examined 93 severe asthma patients. Analysis covered demographic traits, comorbidities, hereditary history, living conditions, exacerbations, past-year hospitalizations, medications, spirometry, and specific IgE presence for 10 aeroallergens.</p> <p><bold><italic>RESULTS</italic></bold><italic>:</italic> <italic>Aspergillus fumigatus</italic> specific IgE notably impacted asthma control. Disease exacerbation risk was 8.4 times higher with <italic>A. fumigatus</italic> sensitization. Multivariate logistic regression revealed higher risk with <italic>A. fumigatus</italic> sensitization (4.79×), fixed obstruction (6.2×), systemic steroids use (5.85×), indoor mold exposure (4.45×), and familial asthma history (2.53×).</p> <p><bold><italic>CONCLUSION</italic></bold><italic>:</italic> <italic>A. fumigatus</italic> sensitization significantly influences asthma course and control, worsening prognosis. Targeted examination of patients measuring sIgE levels for common aeroallergens is necessary to identify control obstacles. The results of the study were previously reported at the All-Russian Congress on Medical Microbiology, Clinical Mycology and Immunology and published in the form of abstracts in the journal “Problems in medical mycology”, 2022, Vol. 24, No 2.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование</bold>. Бронхиальную астму считают одной из актуальных медико-социальных проблем XXI века. В последние годы во всех странах мира отмечен не только рост заболеваемости, но и увеличение распространённости тяжёлых форм бронхиальной астмы.</p> <p><bold>Цель</bold> ― оценить значимость спектра сенсибилизации к аэроаллергенам у взрослых пациентов с тяжёлой бронхиальной астмой как факторов риска неконтролируемого течения заболевания.</p> <p><bold>Материалы и методы</bold>. В обсервационном одноцентровом одномоментном исследовании приняли участие 93 пациента с тяжёлой бронхиальной астмой. В анализ наиболее значимых факторов, негативно влияющих на достижение контролируемого течения болезни, включили качественные и количественные показатели: демографические характеристики; коморбидные заболевания; анамнестические данные о наследственности, бытовых условиях, обострениях и госпитализациях в течение года; объём терапии; результаты спирометрии; наличие специфических иммуноглобулинов E (sIgE) к 10 аэроаллергенам. Определение sIgE в сыворотке крови проводили методом иммуноферментного анализа с использованием тест-системы «АллергоИФА-специфические IgE» и биотинилированных аллергенов производства ООО «Компания Алкор Био» (Россия).</p> <p><bold>Результаты</bold>. Самое выраженное независимое влияние на достижение контроля у пациентов с бронхиальной астмой имело наличие sIgE к <italic>Aspergillus fumigatus</italic>: шансы неконтролируемого течения заболевания при подтверждённой сенсибилизации к <italic>A. fumigatus</italic> увеличивались в 8,4 раза (OR 8,4; 95% ДИ 2,84–24,84; <italic>р</italic> &lt;0,001). Согласно результатам многофакторного логистического регрессионного анализа, шансы неконтролируемого течения болезни статистически значимо увеличивались при наличии совокупности следующих факторов: сенсибилизации к <italic>A. fumigatus</italic> (в 4,79 раза; OR 4,79; 95% ДИ 1,30–17,56; <italic>р</italic>=0,018), фиксированной обструкции (в 6,2 раза; OR 6,2; 95% ДИ 1,99–19,30; <italic>р</italic>=0,0016), при приёме системных глюкокортикоидов (в 5,85 раза; OR 5,85; 95% ДИ 1,17–29,05; <italic>р</italic>=0,031), контакте с плесневыми грибами в помещениях (в 4,45 раза; OR 4,45; 95% ДИ 1,06–18,72; <italic>р</italic>=0,041), отягощённой наследственности по астме (в 2,53 раза; OR 2,53; 95% ДИ 1,02–7,93; <italic>р</italic>=0,047).</p> <p><bold>Заключение</bold>. Сенсибилизация к плесневым грибам рода <italic>Aspergillus</italic> значимо влияет на течение и контроль бронхиальной астмы, ухудшает прогноз заболевания. Для выявления факторов, препятствующих достижению контроля бронхиальной астмы, необходимо целенаправленное обследование пациентов с определением уровня sIgE к наиболее распространённым аэроаллергенам.
