xml:lang="en"><p><bold><underline>Introduction.</underline></bold> Nowadays urticaria is one of the most common diseases. According to the International Guidelines for the definition, classification, diagnosis and management of urticaria, 2<sup>nd</sup>-generation H<sub>1</sub>-antihistamines are recommended to be used as the first-line and second-line therapy. Omalizumab, a humanized monoclonal anti-IgE antibody, is assumed to be the third-line therapy in urticaria treatment.</p> <p><bold><underline>Summary.</underline></bold> In this review we discuss the latest data on pathogenetic mechanisms of urticaria, focusing on the search of the new targets for the therapy. We represent the latest clinical trials of the new biological treatment for urticaria. Safety and efficiency of 4-folds higher therapeutical dose of the 2<sup>nd</sup> generation H<sub>1</sub>-antihistamines, and criteria for personalized selection of the antihistamines are discussed.</p></abstract><trans-abstract xml:lang="ru"><p><bold><underline>Введение.</underline></bold> Крапивница является одним из наиболее часто встречающихся заболеваний. Согласно международным рекомендациям, терапией первой и второй линии крапивницы являются H<sub>1</sub>-антигистаминные препараты 2-го поколения, терапией третьей линии – моноклональное антитело к IgE – омализумаб.</p> <p><bold><underline>Основная часть.</underline></bold> В обзоре обсуждаются патогенетические механизмы крапивницы с позиции поиска новых мишеней для разработки таргетной терапии. Приводятся данные о клинических испытаниях новых биологических препаратов для лечения крапивницы.
